Hospice and home health teams work from cars, kitchens and bedsides, so staff chat has to run on personal phones and still protect patient information. A tool that signs a Business Associate Agreement, organized by care team and with a dedicated on-call channel, gives nurses, aides, social workers and chaplains one shared record of what happened in the home. Family communication stays on the agency's phone lines, not in staff chat.
A team that is never in the same room
A hospice or home health agency of 10 to 15 people might have two RN case managers, an LPN, three or four home health aides or CNAs, a medical social worker, a chaplain, an intake and admissions coordinator, a scheduler, and a clinical manager who also takes on-call shifts. The medical director reviews from a distance. On a normal day none of them are at the office at the same time.
That is why the group text took over. The aide finishes a visit and texts the nurse, the nurse texts the scheduler, the scheduler texts everyone. Each message is about a patient, and each lives on a personal phone the agency has never seen.
Four moments where the team messages
- Change in condition. An aide arrives for a bath visit and finds the patient more confused than yesterday, with less intake. The case manager needs to know before her next visit.
- On-call handoff. The overnight nurse took a 2am call about pain and adjusted a dose per the standing order. The day team needs that context at 8am, not at the next team meeting.
- Visit reshuffle. A nurse is out sick and six visits have to move today, including one patient whose daughter can only be there before noon.
- Interdisciplinary group (IDG) prep. The team's regular review of each patient's plan of care is Thursday, and the social worker needs to know which families have new concerns.
Families stay out of staff chat
Staff chat is for staff. Families call the agency line, talk to the nurse at the bedside, or use whatever patient-facing tool the agency has chosen. Keeping the two separate protects the family from reading team shorthand and protects the team from a message meant for colleagues landing in a daughter's phone. If a chaplain needs to pass along a family's request, it goes into the team channel as a note, not as a forwarded text.
What is PHI in field messages
Protected health information (PHI) is anything that identifies a patient and relates to their health, their care or how it is paid for. 'Mr. A, less intake, more confused, daughter worried' is PHI. So is a photo of a wound or a medication list. 'Delivery company has not confirmed the bed for the 3pm admission' is on the line: no name, but one admission at one time narrows it. A note that the oxygen vendor's phone number changed is not PHI.
A hospice or home health agency that bills Medicare or other payers electronically is a covered entity under HIPAA, so the Privacy and Security Rules apply to the agency directly. Every vendor that stores staff messages is a business associate and must sign a Business Associate Agreement (BAA), the contract that commits it to protect PHI under HIPAA's rules, before handling that data (45 CFR 164.502(e)). WhatsApp, carrier SMS and Facebook Messenger do not sign one.
A channel map for a field team
| Channel | Who is in it | What goes there |
|---|---|---|
| #team-north, #team-south (private) | The nurses, aides, social worker and chaplain for that patient group | Changes in condition, visit notes needing follow-up, family requests |
| #on-call (private) | On-call nurses, clinical manager, case managers | Overnight calls, orders given, what the day team needs to know |
| #scheduling | Scheduler, all field staff | Visit moves, call-outs, mileage questions, no clinical detail |
| #dme-and-supplies | Scheduler, nurses, admissions | Beds, oxygen, wound supplies, vendor delays |
| #admissions (private) | Intake coordinator, clinical manager, case managers | Referrals pending, start-of-care dates, hospital discharges |
| #idg-prep (private) | Whole clinical team | Agenda items for the next team review |
| #office | Everyone | Mileage forms, trainings, holiday schedule |
Mistakes agencies make
- An on-call group text between nurses, with doses and patient names, kept for months on each nurse's phone.
- A WhatsApp group called 'Hospice Team' that the aides prefer because they already use WhatsApp with family. WhatsApp signs no BAA, and an aide who leaves takes the history along.
- Facebook Messenger for the aides' schedule swaps, which absorbs patient names within a week.
- Wound photos sent by iMessage so the nurse can see them before the visit.
Field staff are not careless; they are alone in a driveway with a phone. The replacement has to be as easy as the text. If a phone with any of these threads is lost, the first hour matters; our lost phone guide walks through it.
Setting it up for a 10 to 15 person agency
- The clinical manager creates the workspace and sends invitations to each person's agency email. It is invite-only.
- Create the channels above. Every channel with patient content is private; #scheduling and #office are open.
- Case managers are managers of their team channel; aides, the social worker and the chaplain are members. The scheduler can be a viewer in team channels if she only needs to read.
- Everyone turns on passkeys, since field staff sign in on personal phones, and the 15-minute inactivity lock covers a phone left in a car.
- Post the on-call format in the #on-call topic: time, patient initials, what was reported, what was done, what the day team must do.
- Retire the group text and the WhatsApp group on the same day, with the manager leaving last.
With Yodl the agency pays $10 a month for five users, 20 GB of file storage (voice notes and wound photos included) and a signed BAA, plus $2 a month for each additional person. A 14-person agency pays $28. Push notifications show no message content, and the admin panel has retention and legal hold settings for survey or audit requests.
Yodl, like Slack and Microsoft Teams, encrypts messages in transit and at rest but is not end-to-end encrypted. That is deliberate: it is what lets the manager search, keep retention and export an audit log, which is what a state surveyor will ask for. To get the on-call thread off personal phones, Yodl is $10 a month for five people, with the signed BAA on every plan.
Field teams will always message from the driveway. Give them private team channels and an on-call channel inside a BAA-covered tool, keep families on the agency's lines, and the agency finally owns its own record of what happened in the home.
Frequently asked questions
Can aides use team chat on their own phones?
Yes. The web app runs on any phone, push notifications show no content, passkeys replace typed passwords, and the app locks after 15 minutes. Remove the account the day an aide leaves.
Should families be in the chat?
No. Staff chat is staff-only. Families use the agency's phone lines or its patient-facing tools, and a family request gets written into the team channel by the staff member who received it.
How should on-call be handed off?
In a private #on-call channel with a fixed format: time, patient initials, what was reported, what was done, what the day team must do. The day team reads it before the first visit.
Is a voice note PHI?
If it names or describes a patient, yes. Record it inside the BAA-covered tool, where it is stored and retained like any message, not in a personal messaging app.
